Prime Medical Directors Group
The Medical Director Program pioneers the return of the "house call" physician through a unique and exclusive collaboration with the Prime Medical Director Group. Harkening back to the days when physicians made house calls to visit patients, the Medical Director Program offers our residents convenient access to high-quality, one-on-one, personalized medical attention, within their very own care communities.
We’ve partnered with a team of interdisciplinary doctors within our local service areas to provide in-home care for circumstances that may otherwise have required residents to make unnecessary trips to hospitals and outside providers.
Prime Medical Directors Group consists of: Primary Care Physicians, GI and Nutrition, Neuro-Psych,Podiatry, Dentistry, and Cardiology specialists.
Residents will benefit from:
- a deeper relationship with their doctors
- stronger care coordination
- a focus on preventative treatment and diagnostic services
- better continuity of care
- enhanced communication
- clinical oversight
This ensures a better quality of life throughout every stage of memory care.
Fox Trail Senior Living Communities recognize the enormous responsibility family members embrace when advocating for their loved ones, as well as respect the right to choose their medical providers. All residents have the right to remain under the services and care of their current providers or they may choose to be seen by the PMD group.
Fox Trail Senior Living communities remain committed to excellence and strive to continue our efforts to provide the most innovative and comprehensive care to our residents.
We really like the "home" atmosphere and would seriously consider this house an option for my mother. We really liked the outdoor space and their ideas of engaging my mother in day to day chores and activities. The staff appear to be exceptional in engaging the residents. However, given that the ratio is mostly male and the scheduled activities are more limited than other alternatives, we are carefully considering other options.
